This happened back in November but haven't had a chance to talk about it. Around Thanksgiving I was changing over to my winter tires and while using the jack in the proper position on the front left jack point, it slipped off and the vehicle fell onto the jack. Long story short, the underbody was covered in fluid film and the jack point was slippery. And I was using a rubber spacer on my jack to protect against scratching the unibody. Well rubber and oil equals slippery conditions. So the vehicle slid off the jack and the jack punched into the unibody between the jack point and the pinch weld. Luckily it did not punch all the way through (I think) and just left a huge dent, and also ripped off some of the protective caulk under there. I won't know how bad the damage is until the winter is over, at the time I just sprayed a bunch more fluid film over it.
Also lucky for me, I had loosened all the lug bolts but had not yet started taking them out. If I had taken the wheel off already the jack probably would have ripped right through the floor of my jeep since most of the weight was caught by the wheel still.
And before someone says "should have used a safety jack", this happened within seconds of finishing jacking up the car. I had loosened the lugs before jacking the car, then jacked up the car, then seconds later before I could put in the safety jack it slid off. Moral of the story: make sure that your jack point is free of oil and grease before jacking up.
